Professional services Agreement Forms (PSAs)

After you have been notified by the Owner's Project Manager that your firm has been selected to provide professional services, you will be required to fill out a Professional Services Agreement Proposal Letter.

 

Once you have submitted the proposal letter to the Owner's Project Manager (PM), the Owner will prepare the appropriate Form of Agreement (see agreement templates below) for review and approval. The agreement shall be printed double-sided, signed, and returned to Design & Construction. One (1) fully executed agreement will be returned for your records.

 

Design Professional Agreement Amendments (PSAAs)

If additional services are requested by the Owner's PM after a PSA is executed, an Amendment Letter of Proposal shall be prepared.

 

Once the Amendment Letter of Proposal has been submitted to the Owner's PM, the Owner will prepare a Professional Services Agreement Amendment (PSAA) for the review and approval. The amendment shall printed double-sided,, signed, and returned to Design & Construction. One (1) fully executed amendment will be returned for your records.
